There are several ways to make a bequest to support medical research from your estate. Below are various options for wording your gift, whether (a) unrestricted for OMRF's general use or (b) restricted to a particular research area (sample given).

Specific Bequest - money or property

"I give, devise, and bequeath $ _________or __________(a specific asset) to the Oklahoma Medical Research Foundation (Federal Tax ID 73-0580274), a 501(c)(3) nonprofit organization located at 825 N.E. 13th Street, Oklahoma City, Oklahoma, 73104 (a) for its general use and charitable purposes or (b) to be used for Alzheimer's and other neurological disease or cancer research."

Percentage Bequest

"I give, devise, and bequeath _______% of my estate to the Oklahoma Medical Research Foundation (Federal Tax ID 73-0580274), a 501(c)(3) nonprofit organization located at 825 N.E. 13th Street, Oklahoma City, Oklahoma, 73104 (a) for its general use and charitable purposes or (b) to be used for Alzheimer's and other neurological disease or cancer research."

Combined Percentage and Specific Bequest

"I leave the greater of ____% of my estate or $____________ to the Oklahoma Medical Research Foundation (Federal Tax ID 73-0580274), a 501(c)(3) nonprofit organization located at 825 N.E. 13th Street, Oklahoma City, Oklahoma, 73104 (a) for its general use and charitable purposes or (b) to be used for Alzheimer's and other neurological disease or cancer research."

Residuary Bequest - all or percentage

"I give, devise, and bequeath all or _____% of the rest, residue, and remainder of my estate consisting of property, both real and personal, wherever situated, which I may own or be entitled to at my death, to the Oklahoma Medical Research Foundation (Federal Tax ID 73-0580274), a 501(c)(3) nonprofit organization located at 825 N.E. 13th Street, Oklahoma City, Oklahoma, 73104 (a) for its general use and charitable purposes or (b) to be used for Alzheimer's and other neurological disease or cancer research."
